CLINICAL EXTERNSHIP SURVEYS

Students are required to complete a class evaluation form and an externship student survey so that the experience and training received at Carleen Health Institute can be assessed. We hope to better understand classroom and externship experiences through student participation in the surveys.

Students are also required to complete a Student Population Survey, which is administered to all students enrolled in programs at the institution. The primary purpose of the survey is to obtain student demographic information. This information is confidential and will be used for reporting purposes only and will not, in any way, impact students' academic status. Educational institutions are required to report enrollment data by racial, ethnic and gender categories.
